"Made in Taiwan", "P/N: 141350100100E" and "Rev: 0A" is silkscreened on the board in the FCC photos.
The appearance of this device (or at least some of its shells) is very similar to the ALFA Network AWUS036H. The only major difference is that the RP-SMA connector is absent (a Female USB B port is in its place).
